腹腔鏡下胆嚢摘出術の経験—気腹法と吊り上げ法の比較検討を中心に

Laparoscopic cholecystectomy: Pnemoperitoneum and abdominal wall lifting method

　腹腔鏡下胆嚢摘出術での気腹法と吊り上げ法の利点と欠点について比較検討し，手術の工夫について報告する．術中のconvert症例を除く気腹法，吊り上げ法，それぞれ50例ずつを対象とし，body mass index（BMI），肥満度，手術時間，出血量，術後の在院期間，合併症について比較検討した．手術時間は吊り上げ法のほうが有意に短時間であった．手術において工夫している点は，術中造影を必ず行うこと，胆嚢管は吸収糸にて刺通結紮を含めた二重結紮を行うこと，胆嚢の摘出は手術用のゴム手袋を利用していることである．吊り上げ法は気腹法とほぼ同等の手術が行え，さらに吸引などによる視野の崩れがない点や経済面での利点を有している．視野の面での欠点も慣れれば特に問題はないと思われた．

We compared the advantages and limitations of pneumoperitoneum with those of abdominal wall lifting in laparoscopic cholecystectomy. Our surgical devices are also reported. The subjects were 50 patients who underwent laparoscopic cholecystectomy by pneumoperitoneum and 50 patients who underwent the same procedure by abdominal wall lifting. Those who required conversion during surgery were excluded. The body mass index (BMI), degree of obesity, duration of surgery, volume of blood loss, duration of hospitaliza-tion following surgery, and complications were compared.
