腹腔鏡下虫垂炎手術—標準手術法と手技上の工夫

Laparoscopic operation for acute appendicitis: Standard operation and our technique.

　腹腔鏡下虫垂炎手術は，腹腔鏡下手術共通の利点である低侵襲，小さな皮膚瘢痕など以外にも多くの利点のある手術である．しかし，広く一般的に行われているとは言い難い．緊急手術であり，手術室として，外科医として対応しきれない面があり，また手技の面からも慣れないと敬遠されることがある．われわれは，1991年7月より腹腔鏡下手術を虫垂炎に対する第一選択の手術法としてきた．その経験より，腹腔鏡下虫垂炎手術の標準法となり得ると考える手技と種々の器具を用いての変法，それぞれの方法における工夫，注意点，特徴，欠点などについて報告する．

Laparoscopic operation for acute appendecitis has many benefits such as minimal invasion and good cos-mesis for patients. Neverthless, many surgens have not accepted this operation as standard procedure. The operation for acute appendecitis is often times done in emergency, preparation of the operating room and making the surgeons' will to carry out this procedure difficult. Also, surgeons who have not mastered the technique of laparoscopic appendectomy, are often reluctant to carry out this operation.
