腹腔鏡下虫垂炎手術—手技と治療成績

Laparoscopic surgery for acute appendicitis: A report on 128 operations.

　腹腔鏡下虫垂炎手術施行例128例の手技と臨床成績を検討した．腹腔鏡は臍部から，working portは右側腹部と恥骨直上から挿入され，虫垂間膜はクリップで，虫垂断端は二重結紮で処理された．手術時間は開腹法に比し長いものの，高度の虫垂炎にも対応でき，開腹移行は1例のみであった．トロッカー挿入時の副損傷2例と術後に開腹で対処せざるを得なかった腹壁筋層出血1例が経験した主な合併症で，前者はテンティング防止具を作製して解決し，後者は回避可能なものであった．術後創感染は2例に認めた．本法は安全に広く適応可能で，患者にとっても術創や疼痛の点でメリットは大きく，十分に急性虫垂炎に対する標準術式たり得ると考えられた．

Based on the experience with 128 laparoscopic appen-dectomies for acute appendicitis, the authors present an outline of the procedure and evaluate its results. Under general anesthesia and pneumoperitoneum the opera-tion was performed by 2-working ports on the right flank and the suprapubic area under the laparoscopic vision via umbilicus. Mesoappendiceal vessels were safely controlled by clipping and division method, and appendicular stump simply ligated.
