食道粘膜下腫瘍に対する胸腔鏡下食道切除術

Thoracoscopic Surgery for Esophageal Submucosal Tumor

　食道粘膜下腫瘍に対する治療方針としては，粘膜筋板由来のものには内視鏡的核出術を，固有筋層由来のものには胸腔鏡下核出術を行う．胸腔鏡下腫瘍核出術において視野の作り方は重要であり，baseball diamond conceptsやWakabayashi's conceptsを併用して行う．剥離を腫瘍の周囲のみにとどめればブジーにより腫瘍が起き上がってくることも特記すべき点である．胸腔鏡下手術は，創の痛みや美容面では優れており，とくに良性疾患の手術においては有用な術式であると考えられる．食道粘膜下腫瘍に対する胸腔鏡下手術の手技について解説した．

As a treatment for esophageal submucosal tumor, endoscopic enuculeation is indicated for tumors originating from lamina propria a mucosae, and thoracoscopic enucleation is indicated for tumor occurring from proper muscular layer. To make a good operative field is very important, and this is achieved by using baseball diamonds concepts and Wakabayashi's concepts. The tumor can be lifted up easily by inserting for bougie after dissecting the esophagus around the tumor. The thoracoscopic operation is very useful for benign diseases because of its less invasiveness and good cosmetic reason.
