Is laparoscopic surgery indicated for the treatment of renal carcinomas ?

　腎癌に対する腹腔鏡下手術の是非について検討した．副腎，腎周囲脂肪組織を含めた根治的腎摘除術は可能と考えられるが，腫瘍の摘出は皮膚小切開から一塊として取り出すべきであり，腹腔内で腫瘍を細切する方法は避けるべきと考える．現時点では広範な所属リンパ節郭清は不可能であるため，手術の適応を早期の孤立性小腎癌およびcytoreduction nephrectomyのいずれかに限るべきと考える．炭酸ガス気腹を用いない腹腔鏡補助下腎摘除術は，直視下の腎動静脈の確実な処理，腫瘍の一塊摘出，大動脈周囲リンパ節切除が可能であり，腎癌に対する低侵襲手術の1つとして有用と考えられる．

We reviewed the indication of laparoscopic ne-phrectomy for renal carcinomas. En bloc dissection of the tumor is obtained easily enough by laparoscopic surgery ; however, tissue morcellation for tumor retrieval is not justfied. The tumorous kidney should be removed en bloc together with the adrenal gland, Ge-rota's fascia, and perirenal fat. The indications of laparoscopic nephrectomy should be limited to early, solitary, small tumors (tumor size less than 5-6 cm in diameter) with no hilar lymph node involvement, or to cytoreduction surgery on solitary tumors (less than 7 -8 cm in size).
