Evidence for the Japanese Society for Burn Injuries （JSBI） Clinical Practice Guidelines for Management of Burn Care （3rd Edition）

「熱傷診療ガイドライン〔改訂第3版〕」は入院を要する熱傷患者を対象としたガイドラインで，日本熱傷学会によって2021年に公開された。13領域69のclinical question（CQ）があるが，治療に関する推奨提示は32 CQ（35 Answer）にとどまる。エビデンスに基づいて推奨が示された20 Answerについても，その根拠となったのは小規模な無作為化比較試験（RCT）が多い。重症・広範囲熱傷が少なくなった先進国では大規模なRCTを実施することが困難になっており，今後はレジストリや小規模な観察研究，症例報告などもエビデンスとしてまとめ，診療ガイドラインに反映していくことも必要と考える。

“The Japanese Society for Burn Injuries （JSBI） Clinical Practice Guidelines for Management of Burn Care （3rd Edition）” were published in 2021 by the JSBI for patients who suffered burn injuries requiring hospitalization. There are 69 Clinical Questions （CQs） in 13 areas, but only 32 CQs （35 answers） presented recommendations for treatment. Even for the 20 answers that provided evidence-based recommendations, many were based on small Randomized Controlled Trials （RCTs）. In developed countries where severe and extensive burns are becoming less common, it has become difficult to conduct large-scale RCTs. It will be necessary to compile registries and review small-scale observational studies and case reports as evidence, and reflect them in the practice guidelines in the future.
