Cardiac surgery is expected to improve postoperative survival and functional outcomes with a trade-off of greater surgical invasiveness. While this expectation is fulfilled in many patients who undergo planned surgery, some patients who experience postoperative complications may have a worse prognosis of survival and function than before surgery. The incidence of postoperative complications is highly dependent on the patient's preoperative condition, and the type and number of postoperative complications can affect both short- and long-term postoperative outcomes. In the event of a serious postoperative complication, we should not simply continue to extend treatment with a discrepancy between the patient's expectations and the expectations of the healthcare provider. It is necessary to resolve these discrepancies at key phases of the treatment process based on the information available at the time. If an increase in the frequency of serious postoperative complications and deaths in the ICU is identified, you should consider whether (1) postoperative complications themselves are increasing, (2) the process leading to postoperative complications is being recognized accurately at the bedside, and (3) effective interventions for postoperative complications are being conducted.
