Peripheral ischemia associated with rheumatologic disorders is not common but may be a medical emergency. Peripheral ischemia associated with rheumatologic disorders is often due to involvement of relatively small blood vessels rather than palpable larger blood vessels. When peripheral ischemia is encountered, a complete history and physical examination help intensivists identify possible underlying rheumatologic diseases. When characteristic features of common causes of peripheral ischemia are not apparent, rheumatologic disorders may be important differentials along with other rare etiologies. This article discusses the diagnostic approach to, and management of, rheumatologic disorders with peripheral ischemia including systemic sclerosis, polyarteritis nodosa, and anti-phospholipid syndrome.
