2．慢性呼吸器疾患患者の酸素療法—SpO2の至適目標値

Oxygen therapy in patients with chronic pulmonary disease

近年刊行された急性期治療における酸素療法の2つのガイドラインでは，酸素療法における低酸素血症および過剰な酸素投与による弊害を回避するために，経皮的動脈血酸素飽和度（SpO₂）の目標値を明確にして酸素投与を行うtitrated oxygen therapyの重要性を強調している。慢性閉塞性肺疾患（COPD）をはじめとする慢性呼吸器疾患患者は，特に高二酸化炭素血症のリスクがある。本稿では，慢性呼吸器疾患患者に対する急性期治療におけるtitrated oxygen therapyと，目標となるSpO₂に関するエビデンスを中心に文献レビューを行う。

Main points

●患者が重症であれば，慢性呼吸器疾患の有無にかかわらず，信頼性の高い生体情報が集まるまでは十分な酸素投与がなされる必要がある。

●非重症の慢性呼吸器疾患患者に対する酸素療法は，Ⅱ型呼吸不全のリスクの有無に応じた目標SpO₂を設定し，目標に応じて酸素投与量を調節するtitrated oxygen therapyを意識して行う。

●COPD増悪に対するtitrated oxygen therapyのエビデンスは，複数の観察研究と1つのプレホスピタルのRCTに基づいており，高濃度酸素投与群と比較して高二酸化炭素血症および呼吸性アシドーシスの合併率が低く，死亡率も低いことが報告されている。

●病的肥満，胸郭変形，囊胞性線維症，気管支拡張症，神経筋疾患，オピオイドあるいはベンゾジアゼピン系薬物の過量投与後の患者は，COPD増悪患者と同様にⅡ型呼吸不全のリスク患者として対応するべきである。

To avoid the risk of hypoxemia or hyperoxemia, two recent guidelines for oxygen therapy in adults highlights the importance of titrated oxygen therapy with a specific target for oxygen saturation. Patients with chronic pulmonary disease including chronic obstructive pulmonary disease are vulnerable to oxygen-induced hypercapnia. This article aims to provide a summary of evidence regarding titrated oxygen therapy and appropriate targets for oxygen saturation in patients with chronic pulmonary disease and other conditions with a high risk of hypercapnia.
