中高年期発症の孤発性のレム睡眠行動障害（RBD）は，αシヌクレイン病理を背景に持つことが報告され，長期の経過観察で神経変性疾患であるαシヌクレイノパチーを発症するリスクが高いことが示されている。抽出されたハイリスク群に対して，適切な対応を行うために，ビデオ監視下での睡眠ポリグラフ検査（PSG）によるRBDの確定診断と，患者と家族の背景をもとに将来の人生設計を考慮した予後カウンセリングが必要である。

Abstract

Long-term follow-up studies have shown that middle-aged and elderly people with isolated REM sleep behavior disorder (RBD) have a high risk of developing alpha-synucleinopathy. In order to respond appropriately to these patients, it is necessary to confirm the diagnosis of RBD using video-polysomnography(v-PSG) and provide prognostic counseling that takes into account future life plans based on patient backgrounds and family conditions. This article provides an overview of prognostic counseling for isolated RBD, including a sleep specialist's perspective on disclosing the risks of phenoconversion associated with isolated RBD.
