認知症高齢者への音楽療法に関する研究の動向と看護研究の課題

Research Trends and Issues Recognized by Nurses Regarding Music Therapy among Elderly Persons with Dementia

緒　言

　音楽は人々の心を静穏化させ情動的反応を引き起こすことによる癒しの効果や身体活動を促進する効果があり，人々の健康の維持・増進に用いられてきた。欧米では19世紀から臨床的報告がみられ，20世紀後半から音楽が治療として用いられるようになった。今や臓器移植・遺伝子治療などの先端医療技術は，従来の疾病構造さえも変革しようとしている。しかし，人々の健康あるいは病気の課題は先端医療だけで解決されるものではなく，病気や治療に伴うさまざまな苦しみや痛みに対する全人的なケア，本来の自然治癒力・生命力を回復させるホリスティックなアプローチが必要とされている。近年，欧米を中心に，音楽を健康回復および健康増進だけではなく，病気や障害の治療に使用するようになっている。その適応範囲は，リラクセーション，ストレスマネジメント，リハビリテーションなど情動反応やリズム刺激などを活用した広範囲に及んでいる。

　老年看護の実践場面でも音楽は高齢者の生活の質を高めるアプローチとしてケアに取り入れられている。デイケア，デイサービス，高齢者施設において音楽は生活環境の一部として欠かせないものである。落ち着きのない認知症高齢者も集中して歌や合唱のレクリエーションに参加したり，コミュニケーション障害のある認知症高齢者が歌を通して他者と交流する場面も認められている。音楽療法のほかにも運動，動物，回想などのレクリエーション的アプローチを用いた看護介入は，アクティビティケアと呼ばれて実践に盛んに取り入れられている。そのなかでも音楽は，わが国の高齢者にとっては壮青年期における重要な娯楽であり，共通した情動反応を引き出しやすく，欧米ではアクティビティケアのなかでは最も歴史が長く，研究報告がなされている。今後，わが国でも認知症高齢者に対して介護予防や介護負担軽減を目的した音楽療法を看護介入として活用することは有効であろう。

　Music has been used as a clinical nursing intervention for many years. Clinical research suggests that music may facilitate a reduction in stress response, indicating a decrease in level of agitation. The mid-range theory was conceptual framework of effective music therapy for agitation which of the effects of individualized music intervention provides a framework for nursing research. This paper reviews the efficacy of individualized music-related interventions using mid-range theory to treat agitation in geriatric dementia patients, and also examines published studies of music-therapies on the emotional and behavioral responses of elderly patients with dementia. Experimental setting, methods, evaluations, results and funding sources of selected published studies of music therapy using mid-range theory are discussed. Papers reviewed were classified into five types : (1) reviews of Gardner's mid-range theory, (2) studies of music therapy and agitation, (3) applied research using music therapy, (4) original studies of other types of music therapy utilized in the geriatric dementia population and (5) reviews of research on music therapy in elderly patients with dementia. Future research in this field, using larger and more diverse samples, is necessary. Individualized music interventions are an aspect of nursing care that promote humanistic, individualized care and enhance quality of life. Recommendations for future research include the use of standardized methodology, including the coding of observed behaviors. Music is a potentially effective therapeutic tool in the care of elderly patients with dementia.
