 Increased lactate dehydrogenase (LD, EC 1.1.1.27) activity in serum has been reported in disorders characterized by hyperactive B cell proliferation, especially B cell lymphomas and infectious mononucleosis (IM). However, the cause and significance of the increased LD activity in IM are not entirely clear, and only a few studies have tried to characterize the pattern of LD isoenzymes in serum of patients with IM. It is also unclear from the literature whether LD may serve as a good test for the diagnosis of IM.

 We decided to determine total LD activity and the proportions ofthe LD isoenzymes in sera of patients with IM, to characterize thepatterns of the LD isoenzymes in these patients. We also wanted tocorrelate these findings with the activities of aspartate aminotransferase (AST, EC 2.6.1.1) and alanine aminotransferase (ALT, EC 2.6.1.2)in serum in these patients and to find whether LD activity in serummight be useful in diagnosing IM. Total LD, LD isoenzymes, AST, andALT were determined in 82 consecutive patients with IM.
