  • Look Inside

INTRODUCTION-Nocardiosis is an acute or chronic infection usually manifested in threeforms: 1) mycetoma, a localized cutaneous or subcutaneous tissue involvement*1 of the extremities; 2) pulmonary disease ; and 3) systemic infection. Of the three, pulmonary diseaseoccurs more frequently. In chronic infections, hematogenous spreading*2 leads to systemicmetastatic lesions which often form in the central nervous system. A high mortality rate withundetected or untreated nocardiosis makes early diagnosis and proper therapy essential. Nocardiosis is produced by three species of the genus Nocardia : Nocardia asteroides, Nocardiabrasiliensis, and Nocardia caviae (renamed Nocardia otitidis caviarunt). Rare cases ofinfection have been reported due to N. caviae.*3 N. brasiliensis, which produces mycetomas, occurs more frequently in Central and South America, India, and Africa. N. asteroides oftencauses pulmonary disease and is the most common pathogenic species incurred*4 in the UnitedStates and Europe.
