鉄過剰症の診断とバイオマーカー

Valuable biomarkers for the diagnosis of iron overload

鉄過剰症は文字通り“体内鉄が過剰になった病態”であり，本来肝生検による肝鉄濃度の測定で診断される．しかし生検は大多数の症例で施行困難であり，実際の臨床現場では非侵襲的検査で得られた種々のバイオマーカーが診断に用いられる．血清フェリチン，トランスフェリン飽和度などは診断に必須の血液マーカーであり，MRIによる鉄測定も組織鉄定量に極めて有用である．毒性を発揮する本体と考えられるトランスフェリン非結合鉄(NTBI)も測定法の開発が進んでいる．今後は鉄関連バイオマーカーを適切に評価し，診断・治療に結びつけるなど，医療従事者の鉄過剰症への理解がますます重要になってくるものと考えられる．

Iron overload is diagnosed by quantification of iron in the liver.　However, performing biopsy is not adequate for many patients due to their cytopenias.　Therefore, in clinical practice, alternative biomarkers are utilized for evaluating iron overload.　Serum ferritin and saturation of transferrin are essential markers for the diagnosis, and quantification of tissue iron by MRI is also useful.　Reliable methods for measuring NTBI (non transferrin-bound iron), the essential substance highly reactive to microorgans, have been developed.　For the appropriate diagnosis and treatment of iron overload, we need detailed knowledge about these iron-related biomarkers.
