Biochemical markers of preterm delivery

早産関連の体液を用いた生化学的マーカーには，腟内や頸管の浸出液を用いて行う前期破水の診断マーカーと非破水例に用いる早産マーカーがある．前者はαフェトプロテイン，IGFBP1が代表的マーカーであり，後者には頸管内顆粒球エラスターゼと癌胎児性フィブロネクチンがある．頸管内顆粒球エラスターゼは絨毛膜羊膜炎の前段階の頸管炎の診断に有用であり，癌胎児性フィブロネクチンは絨毛膜に存在することから絨毛膜の炎症や伸展刺激で検出される．進行した絨毛膜羊膜炎では母体血清中のCRPも特異性は低いが有用である．絨毛膜羊膜炎の進行度の判定に使用できるマーカーは，保険適用されていないが，羊水中の顆粒球エラスターゼ，MMP-8などがある．

There are several makers for PPROM (preterm premature rupture of the membrane) and for preterm labor/preterm delivery.　α-fetoprotein and IGFBP1 (insulin like growth factor binding protein-1) of the vaginal fluid are sensitive markers for PPROM.　Cervical granulocyte elastase is a good marker for cervicitis, and onco-fetal fibronectin reflects inflammation of the chorion or damage for the chorioamnion.　CRP of maternal serum is an advanced marker for chorioamnionitis.　Granulocyte elastase and MMP-8 of amnionitic fluid are good markers for staging the degree of chorioamnionitis.
