It is now established that bioactive lipids, including eicosanoids and lysophospholipids, are involved in a broad range of (patho)physiological actions. Determination of their concentrations in human samples such as plasma and serum is important for diagnostic purposes since bioactive lipids mainly act extracellularly. Preparation of clinical samples for laboratory measurement is a difficult task, however, because the blood levels of these lipids can be easily increased by in vitro manipulation after venepuncture. In this review, the current status of the assay of bioactive lipids will be discussed from the clinical viewpoint. Serum thromboxane B2 is suitable for evaluation of aspirin resistance, while autotaxin, which produces lysophosphatidic acid through its lysophospholipase D activity, may be promising as a useful clinical biomarker.
