　HDLは末梢細胞からのコレステロール逆転送を担っており，動脈硬化治療の重要なターゲットである．まず，HDLおよびHDLの主要なアポリポ蛋白であるアポリポ蛋白A-I(アポA-Ⅰ)の特性および機能を解説する．その後，血清HDLコレステロール濃度が低いにもかかわらず動脈硬化が少ない北イタリアの家系で発見されたアポA-Ⅰの変異，アポA-Ⅰミラノの抗動脈硬化作用を概説する．最後に，cholesterol effluxから肝臓へのコレステロールの取り込みまでの複数のステップでコレステロール逆転送に有利に作用し，強力な抗酸化・抗炎症作用を有し，動脈硬化の治療薬として期待されるアポA-Ⅰ擬似ペプチドを紹介する．

HDL plays an important role in reverse cholesterol transport from peripheral cells, therefore, HDL can be a new therapeutic agent for the treatment of atherosclerosis. Here, wa will show the characteristics and functions of HDL and apolipoprotein A-I (apoA-I), a main apolipoprotein of HDL, and will also show an anti-atherogenic properties of an apo A-I Milano, which is mutation found in an Italian family. Further, we will introduce a new therapeutic agent for atherosclerosis, an apo A-I mimetic peptide, which has anti-inflammatory and anti-oxidative effects, and promote cholesterol efflux from peripharal cells.
