 A 29-year-old female sustained a crushing injury to the left side of the face. At that time, she noticed no visual impairment and double vision. Two years later, she suffered from recurrent swelling on the left cheek. X-ray examination including coronal CT revealed a large fracture of the left orbital floor. Inferior rectus muscle and content of the orbit, however, remained intact. Antrotomy showed that the medial portion of the orbital floor fracture prevented drainage from the left antrum and caused recurrent cheek swelling. Coronal CT with variant window levels showed not only the fracture line but the extraocular muscles and ocular adipose tissue. So we recommend use of the coronal CT with variant window levels for the diagnosis of blow out fracture.
