I.はじめに
Frey症候群(味覚性発汗症)とは,食事摂取により主として耳下腺領域皮膚に発来する異常発汗,発赤,灼熱感をいい,別名auriculotemporal syndrome,gustatory sweating and flushing syndromeなどとも呼ばれている。本症は種々の原因で発症するが,耳下腺手術が原因となることが多く,耳下腺手術後の最も不快な合併症として知られている。しかもいったんこの症候群が発症すると極めて難治であり,その治療は困難とされている。
筆者らは,耳下腺手術後に発症した2例の本症候群に,人凍結乾燥硬膜(Lyodura®)を罹患皮下に埋没する方法で治療を試み,良い結果を得たのでここに予報的に報告する。
Frey's syndrome is a relatively common sequelae of parotidectomy. To eliminate the symptoms, various methods of treatment have been reported, but none of them is reliable, effective and long-standing. The authors devised a new surgical approach to improve the symptoms. The principle of the method is to interrupt the connection of parasympathetic fibers to the sweat glands and to interfere with subsequent misdirected reinnervation to those glands by implantation of lyophilized human dura beneath the affected skin of the face. Two patients with Frey's syndrome after parotidectomy were treated by this method, and the following-up studies showed excellent results. Since the procedure is simple and safe, the present method seems to be the treatment of choice.
