Frey症候群の1治験例

A Case of Frey's Syndrome treated with Fascia lata Transplantation

I．はじめに

　Frey症候群は味覚刺激にさいし耳下腺部の皮膚に発汗，発赤，疼痛をみるものである。原因の多くは耳下腺への外科的操作によるものであり，耳下腺手術後の不快な合併症の一つである。発症機構については神経過誤支配説が現在最も支持を得ている。すなわち唾液腺の分泌神経である副交感神経が手術時に切断され，その再生時に誤って近傍の汗腺や皮膚の微細血管を支配するという説^1）である。治療法では神経過誤支配説等の推定される成因にのっとり考按された種々の方法が報告されているが，難治であり確実な方法がないのが現状である。今回われわれは耳下腺良性腫瘍摘出後に発症した1症例に対し，鼓室神経切断術と大腿筋膜皮下挿入術を併用し，良好な成績を収めたのでここに報告する。

We experienced a case of gustatory sweating after complete parotidectomy.The surgical evalua-tion and treatment or gustatory sweating, flushing and cutaneous discomfort, known as Frey's syn-drome, were reported.

We considered that tympanic neurectomy and interpositional fascia graft were the procedures of choice for symptomatic gustatory sweating. These procedures have long-term reliability and is safe. And in terms of reduction of risk and side effects of the procedures we must perform the tympanic neurectomy under operating microscope.
