I.はじめに
欧米においては急性喉頭蓋炎は最緊急を要する疾患の一つと考えられ,主として2〜7歳の小児に好発し成人における発生は稀れであるとされている1,2)。しかしながら本邦においては全く逆の感があり,われわれの経験する症例のほぼ全例が成人例である。この点に関してはいくつかの理由が挙げられる3)が,真相はいまだ謎に包まれているといっても過言ではない。
ただ欧米では小児急性喉頭蓋炎が致死的な疾患として注日され成人においても種々の点で問題となる4)疾患だけに,わが国における本症の実状を調査し把握しておくことは無意味ではない。
Acute epiglottitis is a serious and often fatal condition which must not be missed. The present study revealed that 104 patients with acute epiglottitis had been treated in 42 ENT clinics in Mie prefecture from July 1984 to June 1985. They were 66 males and 38 females, aged 13 to 82 years, though it has been described in European and American literature that the disease occurs mainly in children aged 2 to 7 years. Although a 38-year-old female patient with underlying connective tissue disease required tracheotomy, in the remaining 103 patients antibiotics were effective and both tracheotomy and endotracheal intubation could be avoided. There were no deaths in this series.
