I．はじめに

　喉頭半側切除術（以下半切）後の声門再建法について，われわれは同側胸骨舌骨筋を上方有茎の筋弁として用い，これを直角に折り曲げて声門部に挿入し，その上を粘膜で覆う方法（平野法）を考案して，手術法，術後の機能などについて報告^1〜7）してきた。

　筋弁を被覆する粘膜としては当初有茎下咽頭粘膜か遊離口唇粘膜を用いた。しかし有茎下咽頭粘膜を用いると披裂喉頭蓋ひだが低くなり術後しばらく誤嚥を起こす，粘膜が捻れるので再建した隆起の表面が平面的にならない，粘膜が不規則な振動をする，などの若干の問題がある。一方遊離口唇粘膜はときに壊死を起こすことがある。さらに半切にさいしては患側甲状軟骨板の斜線より前部を健側の前部数mm幅を含めて切除する。そのために術後喉頭の前後径が短くなる。

　This paper describes a modified Hirano technique for glottic reconstruction following hemilaryngectomy and its early functional results. The technique adopts the use of a superiorbased sternohyoid muscle flap and an island cervical skin flap and preservation of a bridge-like segment of the thyroid ala on the affected side. Nine patients underwent this procedure. Their post-operative function was evaluated. The phonatory function was better when the margin of the island skin flap was sutured to the middle of the posterior wall of the larynx than when it was sutured to the lateral wall of the posterior glottis. In the latter cases, there was a marked glottal incompetence during phonation. The glottal incompetence could be corrected by means of a secondary mediofixation procedure.
