遊離小腸移植による口腔再建術

Reconstruction of the Oral Cavity with Free Revascularized Intestinal Graft

I．はじめに

　舌，口腔底腫瘍などの口腔内腫瘍切除後の口腔の再建には，唾液の存在，放射線治療後の血行不良な組織，下顎骨，頸動脈の露出などきわめて困難な問題が多く伴う。そのうえ創の閉鎖のみならず，術後の嚥下，言語など機能面への充分な配慮が必要で，今まで決め手となるような満足な方法はなかったともいえる。最近われわれは微小血管吻合を用いた遊離小腸移植により，9例の口腔再建術を行い，多くの点で満足な結果を得たので，その術式などについて報告する。

　The defects in the oral cavity in 9 cases were reconstructed after ablation of intraoral malignancies with free revascularized intestinal graft.

　As the wall of jejunum is soft, thin and pliable it nicely fits to any difficult intraoral defects.

　Postoperative function was better than those obtained with conventional skin flap methods in terms of deglutition and speech.
