鼻・副鼻腔パピローマの病理学的ならびに臨床的観察

Nasal and Paranasal Papillomas: A Clinicopathological Study of Ten Cases

I．はじめに

　鼻・副鼻腔パピローマは，比較的稀な疾患であるが，易再発性で，時に癌化あるいは癌と共存することがあり，広義の前癌状態ともいうべき，病理学的にも臨床的にも興味深い良性腫瘍である。

　われわれは，かつて鼻・副鼻腔パピローマ5症例について，光顕的，実体顕微鏡的，走査電顕的および透過電顕的に検索し報告した。そのうち2症例に透過電顕的にみて癌と類似した所見があることを指摘しておいた^1）。その1症例が6年後に癌化し，改めて，本腫瘍の診断，治療の重要性を痛感した。本論文は，その後に経験した5症例を加えて，病理所見，とくに透過電顕的所見と臨床経過を対比し，いささかの興味ある知見を得たので報告する。

　A clinicopathological study of nasal and paranasal papillomas was made in 10 cases (4 exophytic papillomas, 3 inverted papillomas, 3 mixed papillomas).

　Using transmissoin electron microscopy, in 4 out of the 10 cases, gaps in the basal lamina were observed in one exophytic papilloma and 3 inverted papillomas. In these 4 cases, infiltration of cytoplasmic processes of tumor cells or of tumor cells themselves through the gaps in the basal lamina into the stroma were observed. One of these 4 cases changed into squamous cell carcinoma 6 years later.

　The pathological findings were not helpful in predicating recurrence.
