鼻出血に対するフィブリン糊を用いた止血方法

I．はじめに

　鼻出血の多くはキーゼルバッハ部位からであり，鼻翼の圧迫，ボスミン付ガーゼ挿入，硝酸銀による腐蝕，電気焼灼などにより比較的簡単に止血しえる。しかし，出血部位が広範囲であったり，深部に認められる場合や拍動性である場合にはベロック・タンポンの使用を余儀なくされる。この際，後鼻孔を閉鎖した上で鼻腔内へタンポンガーゼを挿入するわけであるが，筆若らはタンポンガーゼの代わりに，熱傷後の皮膚移植や整形外科領域などで用いられているフィブリン糊を鼻腔内に充填させることを試みて，タンポンガーゼ挿入以上の止血効果を得たので報告する。

　Fibrin glue consisting of highly concentrated human fibrinogen and thrombin was used for the purpose of hemostasis in 5 cases of severe nose bleeding. These cases were uncontrolled with Bellocq's tampon, and the causes of nose bleeding were one with Rendu-Osler-Weber disease, 3 hypertensions and one chronic hepatitis.
