急性喉頭気管気管支炎について—幼小児の進行性呼吸困難症

　I．緒言

　幼小児に見られる進行性呼吸困難症の中，非ジフテリア性疾患として仮性クループについては一般に広く知られて居るが，仮性クループより遙かに予後不良である急性喉頭気管気管支炎（Laryngotracheobronchitis acuta maligna）に関しては，之が詳細に観察されれば可なり多く存在する疾患であり乍ら，特に本邦では一般に認知されて居ないのは誠に遺憾である。著者等の一人大石^1）は既に雑誌「治療」誌上に急性喉頭気管気管支炎の3例を報告し，本症が本邦ではジフテリア乃至は単なる仮性クループとして取扱われている為治療の面に於て正鵠を欠いて居る事実を指摘したが，最近更にその5例を経験したので茲に再び本症について述べ，大方の御批判を仰ぐ次第である。

　Oishi and Kiuchi report cases of laryngo-tracheo-bronchitis that occurred in infants less than the age 2. In 2 cases the dyspnea was severe and repeated in attacks. The treatment of these cases required the use of antibiotics, tracheotomy, blood transfusions, oxygen and steam inhalations and nebulizers. The use of steroid hormone was effective in relief of dyspnea; the relief of dyspnea required a longer persistence of treatment than that which might be brought on by diphtheria. All 5 cases were diagnosed as cases of diphtheria but none showed the presence of diphtheria bacilli.
