内耳道内に限局した腫瘍の新しいX線検査法—Air CT CisternographyとTarget Imagingの併用法

A New Radiological Examination for Tumor in the Internal Auditory Canal by Combination of Air CT Cisternography and Target Imaging

　I．はじめに

　CTの出現により，聴神経腫瘍をはじめとする小脳橋角部腫瘍に対する神経放射線学的検査の進め方は変化した。新しい世代のCTを使用して経静脈性に造影剤を投与した後のCT（以下IVenhanced CTと略す）をとれば，内耳孔から約1cm以上，後頭蓋窩に突出している場合には，腫瘍を描出することができる。しかし，それ以下の大きさの腫瘍では，CTでの描出は確実ではなく，内耳道内の腫瘍に関しては描出することができなかった。われわれは少量の空気を小脳橋角槽に集めてCT-air CT cisternography（以下air CT-Cと略す）を行ったのちに，コンピュータの演算によって高空間分解能がえられるtarget imagingを併用することによって，内耳道内に限局した腫瘍を描出することができたので，その方法と適用，および小さな聴神経腫瘍に対する神経放射線学的検査の進め方に対して報告する。

　なお，われわれの知るかぎりでは，air CT-Cにて，内耳道内に限局した腫瘍を描出した本邦最初の報告と考えられる。

　A 56-year-old man was admitted to the hospital because of progressive right hearing disturbanceand tinnitus.

　An x-ray film of the skull demonstrated dilatation of the right internal auditory canal. Intravenously enhanced CT didn't reveal any tumor in the right cerebellopontine angle. An intracanalicular tumor was demonstrated by air CT cisternography with target imaging, and confirmed by surgery. This method is useful for the radiological evaluation of the intracanalicular tumors.
