聴神経腫瘍の診断に対するAir CT Cisternography

Air CT Cisternography for Small Acoustic Tumor

I．緒言

　聴神経腫瘍（以下ATと略）の最終的な確定診断法としては，従来より内耳道造影法が行われてきており，とくに少量の油性造影剤と多軌道断層撮影の組合せによる，いわゆるpolytome Pantopaque studyは，小腫瘍の診断には不可欠の診断法とされている。しかしながら，近年のコンピュータ断層撮影（CT）装置はhardおよびsoft warcのめざましい改良，進歩により，かなり小さな空間分解能を有するものも現れ，側頭骨内の小病変の発見，診断には特に威力を期待されている。

　CTを用いてのATの診断はヨード剤静注によるenhancementの併用が，従来より一般に行われてきており，最近においては水溶性の造影剤であるmetrizamideをクモ膜下腔に用いてのCTが行われている段階である。さらに最近ではクモ膜下腔に少量のairを注入して内耳道およびその周囲を造影するair CT cisternography（air CT）がATの診断に用いられ始めている。我々もまたこのair CTをATを疑った症例につき行ってみたので，その診断効果，技術的な諸問題，副作用（合併症），およびその有用性と適応について言及してみた。

　Eighteen patients suspected of having acoustic tumor were evaluated by computerized tomography (CT) with air contrast posterior fossa cisternography (air CT cisternography) combined with target imaging. Air CT cisternography was performed immediately after ordinary CT scans with or without contrast enhancement was done. Fourteen cases were normal with complete air filling of the internal auditory canal. Acoustic tumors were found in three patients. One patient was diagnosed as having stenosis of the internal auditory canal in which air was not filled. There were no serious complications except for headache.

　Air CT cisternography should be indicated especially for patients suspected of having small ear tumor.
