Acinic cell adenocarcinoma of the minor salivary gland origin is common. This 62-year-old man had a tumor in his upper lip. The tumor was widely excised and the defect in the upper lip was repaired immediately by Abbe's method. The size of the defect following tumor excision was about one third of the upper lip. The method was proved to be quite effective in providing excellent aesthetic results. In the histological feature, the tumor was considered as a low grade malignancy, however within one year after operation, regional lymph nodes meta stasis and local recurrence occurred. The authors emphasized that attempts at predicting biologic course based on histomorphologic findings have not been fruitful and the selection of appropriate surgical management is necessary.
