慢性肉芽腫症の1例

CHRONIC GRANULOMATOUS DISEASE, A CASE REPORT

　I．はじめに

　慢性肉芽腫症（chronfc granulomatous disease，CGD）は，白血球機能異常症に含まれ，多核白血球の殺菌能の低下を病因とする疾患である。最近，免疫学の進歩や免疫不全疾患の研究に伴い，小児科領域では注目を集めている疾患の1つである。本症の臨床像は，全身諸臓器の反復する感染と肉芽形成であり，耳鼻咽喉科領域の感染症も部分症として発生する可能性がある。今回われわれは，CGDと考えられる症例で，外耳道の肉芽形成と乳突洞炎をみるに至り，幸いにも乳突洞削開術と抗生物質療法で，局所全身状態とも著明に改善した一症例を経験したのでここに報告する。

　A case of chronic granulomatous disease is reported. A 1.8-year-old boy had suffered from recurrent infections such as cervical lymphadenitis, eczematous dermatitis and perianal abscess in his newborn period. Later he developed left facial palsy, otitis media and intermittent fever. Otoscopic examination revealed a granulomatous mass in the left external auditory meatus, and x-ray showed a diffuse shadow in the left mastoid. Simple mastoidectomy was performed in May 26, 1978. Under intensive postoperative care, the patient has been free from fever and local infection.
