In 16 patients with brainstem dysfunctions, the author studied the possibility of the topographical differential diagnosis in brainstem lesions by auditory brainstem responses.

The results were as follows : 1. Regarding the laterality of brainstem lesions, the findings in auditory brainstem responses were in general accord with the clinical findings. 2. In four patients with midbrain bleeding, auditory brainstem responses showed the evident fifth wave, but remarkably prolonged latency. 3. In ten patients with pontine damages, only the first component was evident to clicks in the mainly affected side and the first three waves were recorded in the unaffected side. 4. In two patients with Wallenberg's syndrome, auditory brainstem responses were normal.
