　I．はじめに

　口蓋扁桃部の腫瘍性腫脹に対しては，ことに健常咽頭組織で被覆されている場合，生検を行なつても確実に組織片を得られず，診断上苦慮するものである。部位的に非上皮性悪性腫瘍が比較的多く発生することから，扁桃の圧排性膨隆として確定診断を得ることなしに治療が先行されることもある。

　私どもの経験した口蓋扁桃部に発生した混合腫瘍は摘出後の病理組織学的検査で確定したものであるが，局所所見から，生検を行なわず，悪性腫瘍であることも考慮し，まず術前照射を行ない，切開術による生検を第一義とし，その段階で摘出可能と判断された場合には全摘に切り変えるという方針の下に手術された症例である。

　A 43-year-old woman presented with a firm smooth swelling on the left tonsillar region.

　On palpation, the swelling was felt deep to the left tonsil and was slightly mobile and had no ballottement as being continuous with the left parotid gland.

　Sialogram of the left parotid gland and the left carotid angiogram showed no abnormality.

　The tumor was readily removed by transoral approach, and it was round, 4 x 3cm in size, and histologically found as a mixed tumor of the salivary gland.
