陳旧性外傷性異物と顔面瘻孔—耳下腺深葉に埋入したガラス片による難治性耳下部瘻孔症例について

FACIAL FISTULA CAUSED BY A LONG RETAINED FOREIGN BODY

　Ⅰ．はじめに

　顔面外傷による異物が組織内に入つた際の障害は異物の種類，埋入の部位と深さによりさまざまである。早期に局所の肉芽腫，または分泌物を伴つた瘻孔形成，肥厚性瘢痕，局所の疼痛をおこすこともあれば，受傷後の長期間無症状に経過し，外傷の病歴を忘れかけた頃になつてはじめて難治性の瘻孔形成の原因となることもある。外傷性異物は外力の大きさとその方向によつては意外に深部にまで異物が達することがある。新鮮例においては受傷時の状況を可及的把握し，深部における異物の有無を確認し，また非定型的な顔面瘻を伴つた症例の治療に際しては異物の存在を念頭においた上で，過去の外傷について詳細な問診を行なう必要がある．顔面皮下組織内に留まる異物と異なり，深部組織たとえば耳下腺深部に埋入した異物が原因となつて炎症がおこつた場合には他の種々の原因による耳下腺炎との鑑別が必要となる。交通外傷の増加に伴い，ガラス片異物はもつともしばしばみられる外傷性異物である。特にガラス片異物は長期間無症状に経過することが多いために，これが原因となつて周囲組織の炎症，瘻孔形成がおこつた場合にはその診断に苦しむことがある。外傷性異物の診断はその存在を疑うことにはじまる。最近経験した陳旧性ガラス片異物症例をもとに診断上の2〜3の問題点について述べる。

　A case of a 7 year-old boy complaining of a persistent infra-auricular fistula with a swelling of the right parotid gland of 5 months' duration is reported. The patient sustained a minor facial injury 3 years ago in which the retroauricular area was cut with a broken piece of the window glass. The wound healing was uneventful and the scar almost disappeared ; the parents had almost forgotten about this accident.

　After 3 years of dormancy, swelling of the parotid gland appeared with persistent purulent discharge from the fistula. Sialography revealed signs of localized chronic parotitis but without any signs of intraductal foreign body or abcess formation communicating with the ductal system.

　Exploratory operation revealed a piece of window glass measuring 25 x 15mm embedded within the deep lobe of the parotid gland encapsulated by dense fibrous tissues. Microscopically, the parotid glands showed a marked infiltration of inflammatory cells and interstitial fibrosis with atrophy of the acini.

　Glass pieces are the most frequently encountered foreign bodies in facial traumas, occasionally causing hypertrophic scar and localized tenderness ; they may be casually found in the secondary revision of the wound without any signs or symptoms.
