Radiotherapy and chemotherapy are primarily the treatment of choice in a case of cancer of the base of the tongue. But, with their poor prognosis of these cases, surgical intervention is sometimes indicated following radio- and chemotherapy.

Direct suturing of the wound after extensive the resection of the base of the tongue and the adjacent pharyngeal wall frequently causes disturbance of movement of the tongue, resulting in articulatory and deglutitive disfunctions.

As movement of the tongue in articulation and deglutition needs much complicated and coordinated action of several muscles, good results cannot be wholly anticipated by transplantation or transposition of the parapharyngeal muscles for correction of the defects in the lingual muscles.

It is important to avoid reconstructing the vallecula and the meso- and hypopharyngeal wall too wide which has a tendency to inhibit intraoral and intrapharyngeal pressure on deglutition.

The flaps we most commonly use in this area are D-P flap, lateral cervical flap, apron flap and forehead flap.

Reconstructive procedures using these flaps were described and their postoperative articulatory and deglutitive functions were evaluated.
