Laryngo-microsurgeryにおける術後疼痛と麻酔管理

MANAGEMENT OF ANESTHESIA IN POSTOPERATIVE PAIN FOLLOWING LARYNGO-MICROSURGERY

I．緒言

　laryngo-microsurgeryを全身麻酔，気管内挿管下で行なつた場合，術後において頸背部を中心とした筋肉痛を訴える症例が多い。その原因として，懸垂頭位による頸部筋群への負荷の他，気管内挿管時に用いる筋弛緩剤succinylcholine chloride静注時の筋搐搦による筋肉痛も関与している^1）-3）。われわれは麻酔剤の選択，筋弛緩剤の投与方法の面から，術後筋肉痛に対する麻酔管理上の対策を検討したので報告する。

　A method by which the severe muscular pain that may ensue following laryngo-microsurgery under intra-tracheal anesthesia may be avoided is considered. First of all, it is highly important to make a selection of anesthetic agent provided with a strong analgesic properties. And secondary, it is also effective to prevent succinylcholine fasciculation with nondepolarizing muscle relaxant. 0.01 mg/kg of pancronium bromide may be administered successfully prior to the administration of succinylcholine chloride without any side effects.
