 It is generally recognized that the morbidity in chronic inflammatory disease such as chronic otitis media, chronic sinusitis and atrophic rhinitis varies with different regions. In order to find an exogenous cause influencing concretely on the development of the disease, an extensive epidemiological study was carried out regarding theregional differences in the incidence of the disease as well as the relationship with which environmental factors hold to them.

 Following conclusions are reached: (1) Chronic otitis media, chronic sinusitis and atrophic rhinitis have an identical distribution epidemiologically in Japan as well as other parts of the world where volcanic region is considered as that of the new age rather than other regions where the latter is considered as old.

 (2) Among other environmental factors such as the climate and water constituents, only atrophic rhinitis was found to be influenced by concentration of silica in the water supply. This finding appears to be consistent with the findings that the concentration of water silica is much higher in the volcanic region of the new age rather than that of the old.

 (3) The amount of silica partaken by people was analysed quantitatively and it was found that epidemiological tendencies paralled that amount.

 Among many environmental elements made study of in disease relationship, it was only the silicon that came to light but further study is highly called for.
