聴器癌における側頭骨全摘出術

TOTAL RESECTION OF THE TEMPORAL BONE IN A CASE OF AURAL CARCINOMA

　I．緒言

　聴器癌は比較的少ない疾患であり，さらに鼓室を中心とするいわゆる中耳癌は稀な疾患であり，早期診断が困難なことも多く，予後不良のことが多いので知られている。その大部分は扁平上皮癌であり，基底細胞癌や腺癌などは非常に頻度が低いとされている。聴器癌に対する病態，分類，臨床像については種々の報告がなされている^1）-5）。一方，治療法についても多く検討がなされており放射線療法，外科的療法，化学療法（局所動注法を含む）およびこれら2者あるいは3者の併用療法が行なわれている。本邦の報告では側頭骨癌に対して中耳根治手術に準ずる腫瘍摘出術が行なわれ，術後に放射線療法を行なつている例が多いがその治癒成績は概して不良である。側頭骨に対する放射線療法に関しては腐骨の発生，脳幹部への障害など，他組織の腫瘍に対するよりもその効果が不良といわれ，また放射線療法後のコントロールされない疾患は腐骨，疼痛で患者を苦しめることになる。いずれにしても手術的療法がとられなければならないが，この場合断片的（piecemeal）に側頭骨病巣を摘出するにとどまらず，腫瘍を含んだ側頭骨全体を摘出することが望ましい。今回自験例および文献的にみられる側頭骨全摘出術について，その手術手技を主として報告する。

　In cases of cancers that may be found to be involving the tympanic cavity and the mastoid region in general, total surgical removal of the temporal bone is regarded as the most effective treatment. However, from the great difficulty with which such surgery is faced, the treatment in most cases rests with surgical removal of the tumor supplmented by administration of anticancerous drugs and irradiation treatments. The patients usually die.

　The authors report a case in point in which they performed successfully, a total removal ofthe temporal bone and describe the procedure with which the internal carotid artery, the facial nerve and the anterior cerebellar artery are approached and disposed ; the replacement of thedura where it was removed by means of fascia lata transplant and replacements of other missing parts by muscular transplants.
