鼻性血栓性海綿静脈洞炎の1剖検例と統計的観察

CAVERNOUS SINUS THROMBOPHLEBITIS, A CASE REPORT

　I．はじめに

　血栓性海綿静脈洞炎（cavernous sinus thrombophlebitis，以下CSTと略す）は，主として鼻腔，咽頭や顔面上半部の化膿性疾患が原発巣となつて惹起されるが，他の頭蓋内合併症に比べてはるかに少なく，われわれが1928年より1972年に至る45年間の本邦文献を調べたかぎりでは27例を集計しえたにすぎない。

　また慢性副鼻腔炎も本症の原因のひとつにあげられているが，この集計の中では1928年細見^1）が副鼻腔炎によると思われるCSTの1例を報告して以来，今日までわずか5例を見るにすぎない。

　われわれは最近，慢性副鼻腔炎よりCSTを併発し不幸な転帰をとつた1例を経験し，剖検する機会をえたので報告するとともに，わが国におけるCSTの統計的観察はこれまで行なわれていないので，集計しえた症例の考察もあわせて試みご参考に供したいと思う。

　A woman, aged 26, was found to be affected with mental confusion, bilateral exophthalmus and high fever. From clinical and x-ray examination of the patient the diagnosis of cavernous thrombophlebitis from chronic purulent paranasal sinusitis was made. The patient was operated by opening the ethmoid sinus which resulted with a copius discharge of purulent material. To this was added intensive chemotherapy. The patient died 5 days after the operation.

　Autopsy revealed meningitis and a pus cavity in the posterior optical fossa.
