若年性鼻咽腔血管線維症例とその治療法の検討

ANGIOFIBROMA OF THE NASOPHARYNX IN YOUNG PERSONS AND THE METHOD OF TREATMENT

　Ⅰ．緒言

　若年性鼻咽腔血管線維腫juvenil nasopharyngeal angiofibroma（鼻咽腔線維腫 Nasenrachenfibrom）は，多くの未解決の問題を含んだ興味ある腫瘍の一つである。特にその治療に関しては統一した考え方は見られないようであり，各治療者は困惑するのが常である。また，古くから本腫瘍と性ホルモンとの関連が云々されているのにもかかわらず，詳しいことはほとんどわかつていない現状である。

　著者らは過去10年間に，本腫瘍9例を経験し，経過観察中の2例を除き7例を全治せしめ得たがその治療に際しては試行錯誤の繰り返しであつたといつて過言ではない。また数年を経て発症した重篤な副作用，後遺症にも遭遇した。

　Nine cases of nasopharyngeal angiofibroma are reported. The authors treated these cases by hormone-therapy in one, which was successful, and the others by surgery combined with irradiation. The following plan of treatment is suggested:

　(1) Radical operation should be the first choice.

　(2) In tumors that have broad basis and where operation is contraindicated, sex-hormone therapy should be given a trial which seem to have a less complication than that of the irradiation.

　(3)) Irradiation therapy should be considered as that of the last resort because of its frequent severe complications.

　The review of literature showed there were 71 cases of nasopharyngeal fibroma reported in this country during the recent 8 years.
