口腔内手術とアセトン尿

INTRAORAL SURGERY AND ACETONUREA

　Ⅰ．緒言

　扁桃摘出術，口蓋成形術などの口腔内操作を行なう手術は，術後の出血，失血性貧血，脱水症状など，術後管理に留意せねばならない事が多い。特に扁摘，アデノイド切除術は，われわれ耳鼻科医が日常，比較的簡単に施行している手術であるが，口蓋に手術侵襲を加える手術で，時にレイリー氏現象によるショック発現を来たす，いわゆる，事故の多い手術である。また，小児では，これらの手術後，一種のショック状態ともいわれる自家中毒症状を起こす事もあり，従来より比較的安易に考えられていた，これらの手術においても，術中，術後の管理には十分なる注意が必要である。

　最近，われわれの教室の手術のうち，比較的多くある小児の口腔内手術の術後に，アセトン尿検査をroutineに行なつて，われわれは術後管理の一助としている。今回，口腔内手術と術後アセトン尿発現に関して，1年間調査したので，ここに報告するとともに，自家中毒症への発展に対する予防，治療，アセトン尿発現の機序についても若干の考察を試みた。

　Postoperative presence of acetonurea was examined in 107 cases of infants operated under general anesthesia for hare-lip, cleft palate and adenotonsillectomies. In each case the age and sex of the patients were note along with the details of the operative procedure and the season in which it was performed. Four cases developed postoperative autointoxication. The mechanism that causes the postoperative acetonurea, prophylaxis and treatment of the autointoxications are discussed.
