聴神経腫瘍の手術経験—特に経迷路法

TRANSLABYRINTHINE APPROACH IN REMOVAL OF AUDITORY NERVE TUMOR

　I．はじめに

　聴神経腫瘍は病理組織学的には良性腫瘍に属するとはいえ，その発生部位の解剖学的関係から，今日でもなお致命的な結果をもたらす腫瘍であり，また頭蓋内に発生する腫瘍のうらではグリオーマ，メニンジオーマについで第三位に下垂体腺腫とほぼ同程度に頻発する脳腫瘍である。神経外科学的にいつても前二者が取り扱い難い腫瘍であるのに反して，聴神経腫瘍はその診断および治療の点で大切な腫瘍ということがてきよう。

　聴神経腫瘍の診断と治療との進歩の過程ほど，尨大な医学史の中でもわれわれの興味をひく一章をかたちづくるものはないと思われるが，この魅力ある物語りの詳細は，最近公表されたモノグラフ"Acoustic Neuromas"（1968）^12）にゆずることとして，ここには少しくふれることとする。

　The operative results of cases of auditory nerve tumor are reported. Six of these cases were operated through translabyrinthine and 1 case through transtemporal approaches.The authors state that the type of operative approach selected should be in accordance to the extent to which the tumor growth may be found particularly in that the tumor may be considered as an ear tumor that of the brain.

　The authors point out the necessity of arriving at an early diagnosis and of performing a careful microsurgery to ratain full function of the facial nerve.
