The surgical treatment of Ménière's dis-ease may be divided into following 3 types of procedures: 1) The release of endolymphatic pressure,

2) A partial destruction of the inner ear and,

3) Operation on the autonomic nervous system.

The first procedure is that of Portmann who introduced it in 1926 and it appears to be the most logical and physiologically satis-factory of them all. In 1951 Yamakawa and Naito introduced a modification of this me-thod by establishing a pathway of drainage of the endolymph into the meningeal space through a window opened in the inner wall of the endolymphatic sac.

For treatment of verigo the modified me-thod has been efffctive in 90.5%; for tinnitus, 57.1% and, for deafness, 28.6%. The author believes that, in the treatment of Ménière's disease by this method, the operation should be performed early in the course of the dise-ase.
