Wegener's Granulomatosis

緒言

　鼻腔，副鼻腔，その他眼上気道において，重篤な進行性壊疽性肉芽性炎症を起こし，その後気管，肺にも同様の病変を起こし，全身の肉芽腫，血管炎を生じ，ついには死に至る疾患は1931年にKlinger^1）がはじめて報告し，ついで1936年と1939年にWegener^2）が詳細に記載してからWegener肉芽腫症と呼ばれるようになつた。いまだ原因不明で適格な治療法もなく，致命的である点で各科に注目されている疾患で，最近では耳鼻科を主として，眼科，内科等に多数報告を見る^3）〜11）。最近著者は，眼瞼に初発して多彩な症状を呈し，臨床的にもWegener肉芽腫症と思われる患者で，初期より死に至るまで観察し剖検した1症例，および耳鼻科にてWegener肉芽腫症と診断され，長期にわたり治療を受けた後，眼症状の現われた2症例を経験したので報告する。

Three cases of Wegener's granulomatosis in-volving the ocular region are reported. In the first case, a female of 57 years of age, the condition started as induration in the upper eyelid to later involve the orbital tissue and the eyeglobe. The lesion developed in the para-nasal tissue. Ten months after appearance of the initial lesion, the patient died from per-foration of the small intestine. In the other two cases, typical eye lesions developed 42 and 51 months respectively after the diagnosis of We-gener's granulomatosis had been established due to signs in the nasal region.
