真珠腫性中耳炎に関する臨床的観察—その1．臨床所見と手術時所見との関係について

CLINICAL OBSERVATIONS ON CHOLETSTEATO-MATOUS OTITIS MEDIA. PART I. THE RELATION OF CLINICAL AND OPERATIVE FINDINGS.

　緒言

　真珠腫性中耳炎は，今日抗生物質を中心とする化学療法や一連の聴力増強手術法の非常な進歩普及にもかかわらず，耳科臨床上しばしば難治の経験をさせられる特異な耳疾患の一つとして取り残されている。本症に特有な骨の破壊的増殖は，もちろん往時に較べてその頻度はいささか減少しつつあるとはいえ，なおいぜんとして諸種の頭蓋内合併症を惹起する危険性を保持している。

　この意味において真珠腫性中耳炎に関して，臨床的観察ことに臨床診断と手術時所見との比較を行うことは，今日なお本症の診断ならびに治療指針上，決して無意味なものではないと信ずる。

From 253 cases of chronic otitis media opera-ted upon within the recent 5 years, 102 cases were recognizable as true cholesteatoma. A comparative review on these cases of true cho-lesteatoma was conducted from the stanpoint of the drum perforation, the situation where the cholesteatomata was localized, of diagnosis and of findings at the time of operation. It was concluded that the diagnosis of cholesteatoma should be made only after a thorough clinical investigation and once a positive diagnosis had been reached the operation should follow im-mediately without any waste of time.
