IMAI reports a case of severe allergic reac-tion provoked in the patient following use of penicillin. Three hundred thousand units of penicillin in oil is instilled in the patients maxillary sinus after the diagnosis of sinusitis was made and confirmed by X-Ray examina-tion. About 10 minutes thereafter, the patient went into a severe anaphylactic reaction but, she recovered. Since this episode about days later the patient was given penicillin aerosol in amount of 10,000 units ; 5 minutes later atfer the conclusion of the treatment another aller-gic reaction but less severe in degree was pro-yoked. In the history of the patient, a woman aged 23, an employee of a bread-factory, an appendectomy had been performed on her about one year previously when penicillin had been given her during the post-operative peri-od. The fact that because of the occupation of the patient being in the bread-factory might have exposed her to biotic growths which may resemble penicillin which might have added to the penicillin administered after the last ope-ration to build up this condition of allergy. At any rate, the author stresses the necessity of making an inquiry as to whether or not the patient is allergic to penicillin before penici-llin in any form is administered to him.
