ステロイドが著効したと考えられるchronic intracerebral hematomaの1例

A Chronic Intracerebral Hematoma for which Corticosteroid was Effective : Case Report

Ⅰ．はじめに

　脳内出血は急激な症状の発現が一般的である．しかし脳内出血の一部には長い期間経過した後，被膜化した占拠性病変としてあたかも脳腫瘍のように徐々に症状を呈するものがある．Yashonら^9）は1978年にこのような症例を“chronic intracere—bral hematoma”として報告した．その後“chronicencapsulated intracerebral hematoma”（Hirshら，1981）^3），“chronic expanding intracerebral hemato—ma”（Pozzatiら，1986）^6）などとして好発部位，経過，画像診断，病理診断，治療などについての報告がなされてきた．今回われわれはステロイドが著効したと考えられる“chronic intracerebral he—matoma”の1例を経験したので，文献的考察を加えて報告する．

Chronic intracerebral hematoma is rare. Computerized tomography almost consistently demonstrates ring-shaped lesions with mass effect and perifocal edema. All patients in published studies have been treated surgically. Most of the hematomas have been encapsulated and contained blood in various stages of organization. The thick capsule has usually consisted of an outer layer of collagenous tissue and an inner layer of granulation tissue.
