開頭術中に応用した超音波断層診断の経験—主として頭部外傷例と脳内出血例

Intraoperative Use of Real-time Ultrasonography in Neruosugery—mainly for head injury and hemorrhage

I．はじめに

　超音波断層診断（以下USと略す）は脳神経外科領域に以前から導入されていて，最近ではBモードを使用し病室や手術室で頭蓋内massの検索に応用されるようになった^2）．われわれはUSのBモードを主として頭部外傷急性期例と脳内出血例の開頭術中に応用したところ，脳内病巣を容易に検索でき手術操作上有用であった．

We used B mode ultrasonography during 61 cranioto-mies performed in the acute stage after head injury, in-tracerebral hemorrhage, ruptured cerebral aneurysm and so on. We examined intracerebral lesions, new hemorrhages near the operative field, and contralateral hemorrhages appearing simultaneously intraoperatively. The resolution with ultrasonography was similar to that of CT and had few obstructing artifacts. It was easy to use and very useful for diagnosing abnormal in-tracranial mass lesions during craniotomy, mainly for the acute stage of head injury or intracerebral hemor-rhage.
