開頭術中の超音波断層法の有用性

The utility of intraoperative ultrasonography in neurosurgical field

1．緒　言

　Aモード表示をもって第三脳室への距離計測に始まった超音波の脳疾患診断法は，装置の機能的変遷と改良に加うるにX線CTの出現によって術中応用^1〜4）という，より実戦的なapproachに変身しつつある。開頭後の超音波による脳断層描出はそれが実時間情報であることから病変の局在の正確な探知を可能とし，またstereotaxicあるいはCT guided approachに代わる新しい，より優れた生検法を生み出したが，本法の歴史がまだきわめて新しいものであるだけに今後さらに多くの利用法が開発される可能性があろう。

　本項ではこの術中超音波断層法の内容を簡単に紹介し，さらに自験例を中心としてその臨床面における有用性を記したいと思う。

Abstract

The intraoperative technique with real-time ultrasonography has been developing in neuro-surgical field in a few years.

This new technique is so useful for the identifi-cation of intracerebral lesions through the dura mater that makes easy and accurate to approachthe lesions for extirpation and/or biopsy. Total experience to date includes 17 patients with brain tumors, one with internal hydrocephalus and one with arachnoid cyst of cervical cord.

In all cases, the lesions were imaged on CRT of the monitor clearly, followed by successful removal or biopsy.
