The Influence of Volume Expansion with Induced Hypertension on Vessel Reactivities, Blood-Brain-Barrier, Cerebral Edema and Infarction

I．はじめに

　Volume expanderにより脳血流を増加させて脳虚血から守ろうとする試みは，くも膜下出血後の脳血管れん縮の際や，手術中の一時的な血流遮断，脳血栓症等の急性期などの際に行われている．実験的研究からは，脳を栄養する大きな動脈間での，脳が本来有している解剖学的なanastomosisを介しての側副血行を増加させることにより脳梗塞の大きさやその後遺症を減じるという報告がある^16,20,21）．そして実際に，臨床の場で急性期の脳虚血発作症例に対し使用され，その有用性も認められている^15,22）．一方，induced hypertensionもまた，多くの病的な状況の結果生じる脳虚血状態を改善させる有効な治療法の一つであると認められており^18），実際われわれも臨床の場で血圧の上昇に伴ない患者の神経症状の改善の得られることを経験することは少なくない．

Eighteen anesthetized cats were randomly divided into 3 groups. One group was treated with 10ml/kg of 6% hydroxyethyl starch (HES). Another group was treated with 15 ml/ kg, and there was a control group. There were six cats in each group. Cerebral pial arterial reactivities were measured by means of intravital mi-croscopic observation through a cranial window. Middle cerebral artery (MCA) was clipped for 70 minutes us-ing a transorbital approach. Five hours later, Evans blue dye was injected intravenously and then the ani-mals were killed.
