I．はじめに

　腰仙部脂肪腫は比較的稀な疾患で，すべての原発性脊髄腫瘍の1％程度といわれている^4）．本症の神経症状発現機序は牽引説^1），圧迫説^4），外傷説^3）に大別されるが，現在ではこの3つの要素が相互に関与していると考えられている^14），その発生は神経管のretrogressive dif—ferentiationの障害により，pluripotentialな性格をもつcaudal cell massの分化により生じる可能性が示唆されている^17,18）．われわれは典型的な腰仙部脂肪腫の1例を経験し，high resolution CT，手術所見および組織学的検討を行い興味ある所見を得たので若干の文献的考察を加え報告する．

A case of 3-month-old girl with lumbosacral lipoma is reported. She had a large soft tissue mass (4×5 cm) in the lumbosacral region initially noted at birth. Inter-pedicular distances below L₂ were dilated on X-P. CT image demonstrated a sharply outlined low den-sity area (approximately-80 H. U.) which occupied the latter half of the spinal canal in the level of L₂ to S₁ level. Defect of vertebral arch was also seen. Lipoma was removed subtotally with laminectomy. CT image demonstrated clear sharp margin of the tumor, neural tissue free zone were not found intraoperatively. Post-operative course was uneventful.
